Hair Breakage or Hair Loss: How to Tell the Difference

Hair in the shower drain tells you almost nothing on its own. What matters is whether those hairs left the follicle or snapped somewhere along their length. One is shedding, which happens to everyone and is governed at the scalp. The other is breakage, which is fiber damage and is governed along the strand. They look identical in the drain and need opposite responses, so the first job is to work out which one you have.

The one-minute test

Pick up several of the fallen hairs and look at the ends under good light.

  • A small white or dark bulb at one end means the hair was released from the follicle. That is shedding, and it is a scalp and follicle question.
  • No bulb, and both ends look cut or frayed means the hair snapped mid-shaft. That is breakage, and it is a fiber question.

Then check length. Shed hairs are mostly full length, because they completed a growth cycle before releasing. Breakage produces short pieces of varying length, often two to ten centimetres, because the strand failed wherever it was weakest.

The third clue is where it happens. Shedding is spread across the scalp. Breakage concentrates where mechanical stress is highest: the mid-lengths, the ends, the section that sits under a ponytail elastic, or the top layer that meets a straightener most often.

What breakage actually is

A hair fiber is a cortex of keratin bundles wrapped in a cuticle of overlapping scales. The cuticle is the load-bearing protection. When it is intact, it holds moisture in, keeps friction low, and shields the cortex. When scales lift, chip and wear away, the cortex is exposed, loses protein and water, and the strand loses its ability to stretch and return.

Damaged hair fails in a recognised pattern. In trichorrhexis nodosa, the classic breakage sign, the cortex splays into a node that looks like two brushes pushed together, and the fiber snaps there. It is a mechanical failure of an already-weakened strand, not a follicle problem (Gavazzoni Dias, Hair cosmetics: an overview, Int J Trichology 2015).

Four forces do the damage, usually together:

  • Mechanical: brushing wet hair, tight styles, friction against fabric.
  • Thermal: heat tools driving water out of the cortex faster than it can be replaced.
  • Chemical: bleach, permanent colour and relaxers, which open the cuticle to work and do not fully close it again.
  • Hygral fatigue: repeated swelling and drying as hair absorbs and loses water, which loosens the cuticle over many cycles.

Wet hair is the vulnerable state. Water swells the fiber and lowers its resistance to stretching, which is why brushing straight out of the shower breaks more hair than the same brushing done dry (Robbins, Chemical and Physical Behavior of Human Hair, Springer).

What shedding actually is

Every follicle cycles through growth, transition and rest. Around 50 to 100 hairs release each day as part of normal turnover, and that number rises when a large group of follicles is pushed into the resting phase at once by illness, surgery, a thyroid change, rapid weight loss or childbirth. Those hairs release together two to three months after the event.

The distinction that matters commercially and practically: no conditioner, mask or bond treatment changes shedding, because shedding is decided inside the follicle. And no scalp serum repairs a snapped fiber, because that damage sits in dead keratin.

Both at once, which is common

Breakage and shedding are not exclusive. Hair that is thinning from the follicle is also finer, and finer hair breaks more readily under the same brushing and heat. People often read the combination as accelerating hair loss when part of it is damage stacked on top.

The test still works. Sort a handful of fallen hairs into bulbed and unbulbed, and the ratio tells you where to put the effort. Mostly bulbed means work at the scalp. Mostly unbulbed means work on the fiber. A real mix means both, and the scalp work should start first because it governs what grows next.

Protein and moisture, and why more protein is not the answer

Breakage is often met with protein treatments. Protein does have a role: hydrolysed keratin and collagen fragments are small enough to deposit into gaps in a damaged cuticle and temporarily restore stiffness and tensile strength. But hair needs a balance of protein and moisture, and too much protein leaves the fiber rigid and brittle, which breaks in a different way.

The signs are distinguishable. Protein-heavy hair feels rough, straw-like and stiff, and snaps cleanly. Moisture-heavy hair feels limp and mushy, stretches a long way when wet and does not spring back. Our guide to the protein and moisture balance sets out how to read yours and correct it.

What reduces breakage

Fiber damage cannot be reversed. Keratin is not living tissue and does not heal, so every intact strand you keep is one you protected rather than repaired. The work is in reducing the load and reinforcing what remains.

Reduce the mechanical load. Detangle from the ends upward, use a wide-tooth comb on wet hair or wait until it is damp, and swap tight elastics for soft ties. Change where the tie sits so the same section is not always stressed.

Reduce the thermal load. Lower the tool temperature and reduce frequency. Heat damage is cumulative and each pass adds to it.

Reinforce the cuticle and hold moisture. Ceramides are the lipids that sit between cuticle scales and seal the barrier. Replacing them reduces water loss and lowers friction, which is what makes the fiber survive handling. Treat the scalp, because it decides the next strand. A strand's strength is set while it is being built. When to see a doctor rather than change products

Take it to a dermatologist if shedding is heavy for more than three months, if you can see scalp through the hair where you could not before, if patches are smooth and round, or if there is pain, scaling or scarring. Scarring alopecias destroy the follicle permanently and are time-sensitive; the earlier they are treated, the more is preserved. A cosmetic routine supports the scalp environment and the fiber.
